Local Doctor Leaving Clinic for Solo Practice

Dr. Jay Connelley, longtime physician at Monticello Medical Clinic, will be leaving the organization, effective January 1, to establish his solo practice.
The new clinic, Connelley Family Medical, will be adjacent to Reinhart FamilyHealthcare on Jordan Drive. Although neighbors, each clinic will have separate staff, entrances and office operations.
