Brenda Jacobs Wins National Award
December 5th, 2019 by SubmittedDePaul Community Health Centers is pleased to announce receipt of recognition from the National Committee of Quality Assurance(NCQA) as a Patient Centered Medical Home.
The National Committee for Quality Assurance is an independent 501 nonprofit organization in the United States that works to improve health care quality through the administration of evidence-based standards, measures, programs, and accreditation. The NCQA Patient Centered Medical Home Recognition Program provides health care practices information about organizing care around patients, working in teams, and coordinating and tracking care over time. DePaul Community Health Centers under the leadership of Brenda Jacobs DNP FNP-BC has been working on practice transformation for the last 18 months. This recognition is a stamp that indicates we are providing the best quality when caring for our patients. DePaul community Health Centers will be required to report quality metrics annually to retain this status.
It is an honor to be considered as a Patient Centered Medical Home, aligning with our mission of holistic care or taking care of the “whole” patient. Soon, we will have full integration of Behavioral Health Services as well. A telehealth program is also being added to address behavioral health needs within the area. We currently offer medical, dental and social services. We have a state of the art wellness center located in Gould, Arkansas.
Our Dumas and Gould clinics are certified rural health clinics. Our Medical Director, Dr. Robert Scott has been a champion for the practice transformation. The Patient Centered Medical Home model ensures that every team member practices at the top of their license, making the visit more efficient and timely. Jacobs reports that “we strive to make every patient visit a positive experience.” Other services provided include Chronic Care Management. This program allows us to make telephone calls to patients, designated family members. Medical equipment companies, ad pharmacies to coordinate the patient’s care.
